When scheduling an initial evaluation a deposit of $350.00 is required*. The deposit must be made six weeks before your evaluation. All deposits are deposited six weeks prior to the first day of evaluations at your local branch. The remaining balance is due at your evaluation.

When Trimester Requirements Have Been Fulfilled

When Trimester Requirements Have Not Been Fulfilled

Reevaluations are made when you are at your current evaluation. Evaluations are held every four months, and a $250.00** deposit Is required to hold your spot on the schedule. If evaluations are not fulfilled every four months the fee increases to the listed price under: When Trimester Requirements Have Not Been Fulfilled.
